APPLICATION FOR PRELIMINARY OFFICIAL

ACTION AND RESERVATION OF BOND LIMIT

 The purpose of this application is to provide a brief description of the proposed project and financing in order to assist the Business Finance Authority (the "BFA") in determining whether the project appears eligible for financing through an issue of bonds by the BFA and, if so, whether it should be so financed.

Please answer all questions or insert N/A where questions do not apply, and submit this application along with each of the following items, if applicable. We ask that you provide twenty (20) copies of any bound or printed materials.

(1) For all projects:

1. the applicant's annual report, brochure or other printed material describing the applicant;
2. the applicant's most recent available annual financial statements, or other financial information about the applicant; and
3. nonrefundable application fee.

(2) For projects for Small Scale Power Facilities, Water facilities and Water Powered Electric Generating Facilities, a feasibility study;

(3) For projects for Small Scale Power Facilities and Water Powered Electric Generating Facilities, letters of intent to purchase electricity generated by the facility from proposed purchasers; and

(4) For waste disposal projects, letters of intent to supply waste material from the proposed suppliers of such material, and if such material is being converted into other products, such as steam or electricity, letters of intent to purchase such other products from the proposed purchasers.
